Below is information on getting passports:

New Passports: Use form DS-11 - Application for a U.S. Passport

Form DS-11:
Needed Items:	An original birth certificate, with an additional copy of birth certificate.
	Front and back copies of your Driver's License.
	Children must be there if applying and both parents present.
	For each child under 16 a set of each parents drivers license copied both front and back.
	Photo
	Must be paid with 2 personal checks.

Renewal of a Passport: Use form DS-82 - U.S. Passport Renewal Application for Eligible Individuals

Form DS-82:
Needed Items:	Front and back copies of your Driver's License.
	Expired Passport
	Must be paid with 2 personal checks.

Notes:

• For the DS-11 and DS-82 forms noted above, go to the website https://travel.state.gov/content/passports/en/passports/forms.html
  
• All applications must be typed from web page or filled out in black ink only. Do not use blue ink, and do not sign them.
• Passports require two separate fees which must be paid for using different checks.
• Processing times are generally 6 to 8 weeks for routine applications and 2 to 3 weeks for expedited applications.
